Including it in an oral health supplement is a deliberate choice that reflects how the upper airway and oral cavity share a continuous mucosal surface. This article unpacks what BL-04 does, what the trials show, and why it earns a spot in the formula.

What BL-04 Is

Bifidobacterium animalis subsp. lactis is a Gram-positive, anaerobic, rod-shaped probiotic species widely used in dairy fermentation and dietary supplements. The BL-04 designation refers to a specific strain owned and characterized by Danisco/DuPont. It was deposited in the Belgian Coordinated Collection of Microorganisms in the late 1990s and has been the subject of more than two dozen human clinical trials since then. The strain is notable for surviving stomach acid and bile better than many bifidobacteria, which historically limited their viability in supplements.

Why an Immune-Focused Probiotic in an Oral Product

The mouth, throat, nasopharynx, and sinuses are connected by continuous mucous membranes, share lymphoid tissue (the Waldeyer's ring of tonsillar tissue), and host overlapping microbial communities. Oral pathogens regularly seed the throat; respiratory pathogens regularly land in the mouth. BL-04 specifically has been shown to modulate certain immune markers (most notably secretory IgA and natural killer cell activity) and to reduce the incidence of upper respiratory tract infections in active adults. A separate experimental rhinovirus challenge study (Turner et al. 2017) examined whether BL-04 could reduce symptom severity following deliberate viral inoculation; results were modest but pointed in the right direction.

How BL-04 Interacts with the Oral Cavity

BL-04 is not a classical periodontal-focused probiotic. It does not have the same documented anti-gingivitis effects as L. reuteri or the same anti-cariogenic effects as some L. paracasei strains. What it appears to do in the mouth-throat axis is help maintain a balanced anaerobic population in the posterior tongue and tonsillar crypts. This matters because the tongue's posterior dorsum is the single largest reservoir of bacteria in the oral cavity, including most of the volatile sulfur-compound producers responsible for chronic bad breath. Why Strain ID Matters Here Too

As with L. paracasei, the species name B. animalis subsp. lactis covers many strains, and their clinical profiles diverge. BB-12, HN019, Bi-07, and BL-04 are all subsp. lactis strains, all sold commercially, and each has its own published research base. Studies on BB-12 cannot be assumed to apply to BL-04 even though they share a subspecies designation. ProDentim's label specifies BL-04, which is the appropriate practice and lets researchers and clinicians match the product to the published evidence.

Dose and Delivery

Published BL-04 trials have generally used daily doses in the range of 1 to 4 billion CFU, taken once daily for periods of weeks to months.
